I tre dagar har jag f\u00e5tt lyssna till framst\u00e5ende presentationer om nul\u00e4get, visioner om framtiden och aktuell forskning kring skolfr\u00e5nvaro\/n\u00e4rvaro. Vid sidan om det vetenskapliga har jag ocks\u00e5 njutit av den stormiga Atlanten, l\u00e5nga promenader l\u00e4ngs den milsl\u00e5nga sandstranden, middagar, trevliga samtal och m\u00e4nniskor.<\/p>\n\n\n\n<p>Jag vill skriva om mina tankar om behandling av skolfr\u00e5nvaroproblem. Nu \u00e4r det ju inte skolfr\u00e5nvaro per se som man traditionellt riktat in behandlingen p\u00e5, utan t.ex. \u00e5ngest eller depression som man sett som orsak till den. Det blir dock vanligare att anv\u00e4nda insatser som \u00e4r specifikt inriktade p\u00e5 att hj\u00e4lpa elever till skolan och att n\u00e4rvara i skolan trots \u00e5ngest, l\u00e5g motivation etc. Dessa insatser kan vara riktade till den enskilda eleven, men omfattar oftast ocks\u00e5 familjen (f\u00f6r\u00e4ldrast\u00f6d och handledning), skolan (hur ser skolsituationen ut f\u00f6r eleven) och vardagen i stort (social situation, mobbning mm.).<\/p>\n\n\n\n<p>Orsaker till fr\u00e5nvaroproblem kan grovt delas in i individ-, familjerelaterade, sociala och skolrelaterade faktorer, samt samh\u00e4llsfaktorer. Individfaktorer kan vara t.ex. ett \u00e4ngsligt temperament (eller \u00e5ngest), inl\u00e4rningssv\u00e5righeter eller ett ut\u00e5tagerande beteende. Familjerelaterade faktorer kan handla om familjens attityder till skolg\u00e5ng, sv\u00e5righeter f\u00f6r f\u00f6r\u00e4ldrarna med gr\u00e4nss\u00e4ttning eller att st\u00f6da sitt barn med \u00e5ngest till skolan. Sociala faktorer handlar om relationer i skolan, om man har kompisar, ensamhet eller mobbning. Skolfaktorer handlar om t.ex. skolans klimat, de fysiska f\u00f6ruts\u00e4ttningarna, hur man p\u00e5 skolniv\u00e5 f\u00f6ljer med och tacklar mobbning, v\u00e4lm\u00e5ende och fr\u00e5nvaro. Med skolk avser man fr\u00e5nvaro d\u00e5 man antar att eleven g\u00f6r n\u00e5got mer motiverande utanf\u00f6r skolan och d\u00e5 skola och\/eller f\u00f6r\u00e4ldrar inte godk\u00e4nt fr\u00e5nvaron. Grovt sett kan man kategorisera insatser vid skolk som s\u00e5dana som \u00f6kar p\u00e5 elevens engagemang f\u00f6r skolan, eller s\u00e5dana som har bestraffning som metod. Med engagemang avser man elevens tankar och k\u00e4nslor gentemot skolan. I en \u00f6versiktsstudie j\u00e4mf\u00f6rde man dessa tv\u00e5 metoders effektivitet (Keppens &amp; Spruty, 2020). F\u00f6ga \u00f6verraskande kom de fram till att interventioner baserade p\u00e5 bestraffning s\u00e4llan fungerade, medan s\u00e5dana d\u00e4r man str\u00e4vade efter att \u00f6ka elevens engagemang gav positiva resultat. Det som \u00e4r intressant i detta sammanhang \u00e4r att d\u00e5 vi talar om insatser f\u00f6r skolk handlar dessa om att motivera eleven till skolvardagen genom att \u00f6ka gemenskapsk\u00e4nsla i skolan, se \u00f6ver st\u00f6dformer m.m. Dessa \u00e4r insatser som till stor del g\u00f6rs inom skolan, \u00e4ven om st\u00f6d f\u00f6r familjen ocks\u00e5 finns med. Nu vet jag de facto inte hur situationen i Finland ser ut g\u00e4llande insatser f\u00f6r skolk \u2013 hur man i praktiken g\u00f6r d\u00e5 eleven skolkar mycket. Studier har visat att behandlingen ofta ger positiva resultat. \u00d6verlag g\u00e4ller dock att ju sv\u00e5rare problematiken \u00e4r, desto sv\u00e5rare \u00e4r det att \u00e5terg\u00e5 till n\u00e4rvaro. S\u00e5 elever med komplex och kronisk fr\u00e5nvaro \u00e5terg\u00e5r ofta till skolg\u00e5ng deltid eller med olika st\u00f6dformer klara av att delta i undervisning t.ex. p\u00e5 distans. En generell princip \u00e4r att ju tidigare man ingriper, desto mer \u00f6kar n\u00e4rvaron.<\/p>\n\n\n\n<blockquote class=\"wp-block-quote is-layout-flow wp-block-quote-is-layout-flow\"><p>En generell princip \u00e4r att ju tidigare man ingriper, desto mer \u00f6kar n\u00e4rvaron.<\/p><\/blockquote>\n\n\n\n<p>F\u00f6r elever med skolv\u00e4gran g\u00e4ller ofta att man kan \u00e5terg\u00e5 till skolan, men att man forts\u00e4ttningsvis har h\u00f6ga niv\u00e5er av \u00e5ngest. I sin f\u00f6rel\u00e4sning p\u00e5 konferensen pratade amerikanske Ann-Mari Albano om behandlingseffekter f\u00f6r \u00e5ngest. Hon visade ett diagram baserat p\u00e5 data fr\u00e5n det amerikanska sk. CAMELS-studien (Ginsbury et al. 2018). Av de som behandlats var enbart ca 20 % symptomfria efter fyra \u00e5r, de som initialt f\u00e5tt hj\u00e4lp av behandlingen till n\u00e5got h\u00f6gre grad \u00e4n de som inte \u00f6verhuvudtaget f\u00e5tt hj\u00e4lp av behandling.&nbsp; N\u00e4stan 50 % fick \u00e5terfall och f\u00f6r n\u00e4rmare 30 % blev problematiken kronisk.<\/p>\n\n\n\n<p>Mer positiva resultat rapporteras i den danska Back2School interventionens pilotstudie. 24 familjer deltog i studien d\u00e4r man uttryckligen behandlade fr\u00e5nvaroproblem. Vid start var fr\u00e5nvaroprocenten 67 % och vid avslutad behandling 26 %. Efter ett \u00e5r var den 20 %. En stor del elever var tillbaka till fulltid i skolan, medan fyra elever deltog mindre \u00e4n 50 % och en inte \u00f6verhuvudtaget. \u00c4ven i denna studie framst\u00e5r det komplexa och problematiska med just denna typs problematik, det \u00e4r helt enkelt sv\u00e5rt att hj\u00e4lpa alla med olika typer av fr\u00e5nvaroproblem.&nbsp;&nbsp;<\/p>\n\n\n\n<p>Dessa data \u00e4r inte positiva. Nu kan det finnas andra studier med b\u00e4ttre utfall, men jag anv\u00e4nder dessa som utg\u00e5ngspunkt f\u00f6r mitt resonemang. N\u00e4r det handlar om skolfr\u00e5nvaro, har vi f\u00e5 studier som har l\u00e4ngre uppf\u00f6ljningstid \u00e4n sex m\u00e5nader (Back2School pilotstudien ett undantag). Och d\u00e5 har man konstaterat att \u00e5ngestniv\u00e5n forts\u00e4ttningsvis \u00e4r h\u00f6g. Fr\u00e5gan \u00e4r: vad h\u00e4nder med dessa elever efter sex m\u00e5nader?<\/p>\n\n\n\n<p>Svaret \u00e4r att vi inte vet. P\u00e5 basen av Albanos presentation skulle jag gissa p\u00e5 att v\u00e4ldigt m\u00e5nga forts\u00e4ttningsvis tampas med \u00e5ngest efter en tid. Beroende p\u00e5 problematiken som orsakat skolv\u00e4gran finns det s\u00e4kert olika utfall. Po\u00e4ngen jag vill lyfta fram \u00e4r att vi beh\u00f6ver uppf\u00f6ljningstider i studierna som \u00e4r l\u00e4ngre \u00e4n sex m\u00e5nader. Vi beh\u00f6ver f\u00f6rst\u00e5 vad som orsakar \u00e5terfall, vad som g\u00f6r problematiken best\u00e5ende. Vi beh\u00f6ver se \u00f6ver v\u00e5ra st\u00f6dformer s\u00e5 att de blir mer effektiva ocks\u00e5 p\u00e5 l\u00e5ng sikt.<\/p>\n\n\n\n<p>I Finland beh\u00f6ver vi dock b\u00f6rja med att se p\u00e5 effekterna av interventioner \u00f6verhuvudtaget \u2013 de enda psykosociala interventionerna f\u00f6r specifikt skolfr\u00e5nvaro jag k\u00e4nner till \u00e4r sedan n\u00e5gra \u00e5r Back2School i v\u00e4stra Nyland (se <a href=\"https:\/\/www.koulukunnossa.fi\/\">koulukunnossa.fi<\/a>) och Kotoa Kouluun i \u00c5bo (se <a href=\"https:\/\/vslj.fi\/kotoa-kouluun\/\">https:\/\/vslj.fi\/kotoa-kouluun\/<\/a>).
